Formbox is a standalone FHIR-native form building and structured data capturing platform by Health Samurai⁠.

⁠Run locally

Download and up docker-compose.yaml file from Aidbox user portal

mkdir formbox && cd formbox
curl -JO https://aidbox.app/runme/formbox
docker compose up

Open in browser http://localhost:8080⁠ and activate your Formbox instance using AidboxID⁠

Press the button Forms in the console in browser (or by visiting http://localhost:8080/ui/sdc⁠)
